Aspergillus Endocarditis in a prosthetic Valve following COVID-19 Infection

چکیده
Endocarditis of a prosthetic valve is known but rare occurrence. Fungal endocarditis extremely rare. However, in the COVID-19 pandemic, there has been increasing recognition fungal or yeast-related infections, often due to combination immune paresis, concurrent use corticosteroids underlying chronic conditions such as diabetes neoplasms. We present patient with fulminating, invasive aspergillosis aortic who presented catastrophic cardiovascular failure needing intensive care management. Early and prompt therapy cornerstone success presentations high mortality.

Aspergillus prosthetic valve endocarditis.
The clinical, laboratory, and histopathological features of seven cases of Aspergillus fumigatus prosthetic valve endocarditis are presented. The exact nature of the lesion, a combination of infective fungal endocarditis and thrombosis on the prosthetic valve, is discussed and the difficulties in clinical diagnosis are emphasized. Helpful indications were sudden unexplained heart failure with t...

A 74-year-old male presented to the emergency department 46 days after undergoing an aortic valve replacement. He presented with nonspecific symptoms developed over the previous 15 days, with a new onset of a systolic panfocal murmur. Echocardiography revealed a great vegetation measuring 15 mm by 23 mm causing a severe obstruction of the bioprosthesis. The patient underwent an emergency surgic...

In 38 cases of prosthetic valve endocarditis, 19 were early cases (onset < 60 days after insertion of prosthesis) and 19, late cases (onset> 60 days). Nine late cases had onsets 12 to 53 months after surgery. The sources or predisposing factors in late cases included dental disease or manipulation; genitourinary tract procedures; and skin, urinary, or wound infections. In contrast, most early c...

Prosthetic valve endocarditis occurs in 2-4% of patients with prosthetic valves and carries a mortality of around 50%. A low index of suspicion should enable early diagnosis. The empirical use of antibiotics in patients with a prosthetic valve who develop an unexplained fever is inappropriate. Careful liaison between cardiologist, microbiologist and cardiac surgeon is essential for optimal mana...

Fifty-three (3.6%; actuarially 4.1% at 48 months) of 1465 consecutive in-hospital survivors of valve replacement from 1975 to July 1979 (aortic, mitral, or aortic and mitral, only one untraced) developed prosthetic valve endocarditis (PVE). Incremental risk factors for developing PVE were native valve endocarditis (p < .0001), black race (p = .0001), mechanical prosthesis (vs bioprosthesis) (p ...
